Scheduling your first appointment
Please allow us at least one business day to respond to your appointment request, though typically we will strive to respond on the same day. It is recommended to schedule your first visit one week in advance.
If your pet has previous medical records, please attach them in the form below or bring them with you to your visit.
If you are bringing in your pet for a sick visit, please include information about when the symptoms began.
New clients are welcome to call our front desk at

Please download and fill out the Intake Form before your first appointment. You can then attach it to the New Client form on this page or print and bring with you.

Having an Emergency?
If your pet is having an emergency, please call our office directly at 718-624-1200 or take your pet to an Emergency 24 hour hospital such as VERG North or Blue Pearl Brooklyn immediately.

(Food & medication pick up until 6:00pm)
Saturday: 9:00am - 3:00pm
(Food & medication pick up until 5:00pm)
Sunday: 10:00am – 4:00pm
(Open only for food & medication pick up)
If you have a concern about an expiring vaccine, please contact us.
The office will be closed on Thanksgiving Day, Christmas Day, New Year's Day, Easter Sunday, July 4th and Memorial Day.
